Is there a way to measure someone's level of consciousness through a survey?

Hi all, is there a known way to measure someone’s center of gravity in terms of level of consciousness through a simple questionnaire?

I’m organizing a European Citizens Initiative, a petition to get European legislation changed, and I’m interested if we can tailor our messaging to different groups and on different platforms based on what level of consciousness a person gravitates towards.

I would be grateful for any suggestions.

Not sure about a simple survey but sentence completion tests like the Maturity Assessment Profile and/or the ones from StagesInternational.com could work
